Transition of care from cure to palliation This section focuses on the transition of care from disease modifying or curative treatment to supportive and palliative care. The difficulties that this transition poses for all involved (patients, family and caregivers and the treating health care team) are discussed. Patterns of disease and decline with associated points of transition are also illustrated.
