Referring to specialist palliative care services

Patients facing a life limiting illness are cared for across inpatient and community settings. Many patients' care needs will be well catered for by their primary health care providers. Some patients or their family and/or caregivers will have identified complex needs, which require the input, expertise or coordination of the specialist team. Guidelines for referrals and prompts for identifying complex needs are discussed.
